ASSUMPTION OF RISK: I, the undersigned, acknowledge that participating in the escape room activities at Escape Orbix (operated by Gib Investment Group INC) involves inherent risks including but not limited to: physical injury, property damage, emotional distress, and in extreme cases, death. I voluntarily assume all such risks.
RELEASE OF LIABILITY: In consideration of being permitted to participate in Escape Orbix activities, I hereby release, waive, discharge, and covenant not to sue Escape Orbix, Gib Investment Group INC, its owners, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained while participating in any activity or while on or about the premises.
INDEMNIFICATION: I agree to indemnify and hold harmless Escape Orbix and Gib Investment Group INC from any loss, liability, damage, or costs they may incur due to my participation in any activities or presence at the facility, whether caused by negligence or otherwise.
RULES & CONDUCT: I agree to follow all posted rules, staff instructions, and safety guidelines at all times. I understand that failure to comply may result in immediate removal from the premises without refund. I agree not to use excessive force, damage property, or engage in unsafe behavior.
HEALTH CERTIFICATION: I certify that I am in good physical health and have no conditions that would prevent safe participation. I understand that Escape Orbix staff are not medical professionals and cannot provide medical advice or treatment.
PROPERTY & PERSONAL BELONGINGS: Escape Orbix is not responsible for lost, stolen, or damaged personal property. I agree to secure my belongings before entering any game room.
RIGHT TO REFUSE: Escape Orbix reserves the right to refuse or terminate participation of any individual at any time for any reason, including but not limited to: intoxication, disruptive behavior, or violation of rules.
GOVERNING LAW: This agreement shall be governed by and construed in accordance with the laws of the state in which Escape Orbix operates. Any disputes arising under this agreement shall be resolved through binding arbitration.
SEVERABILITY: If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
ACKNOWLEDGMENT: I confirm that I have read this entire document, I understand its contents, and I am signing it freely and voluntarily. I am at least 18 years of age or am the parent/legal guardian signing on behalf of a minor.
